Do I need a referral or prescription for PT from my physician??

No, you do not need a referral from your physician to be seen by a physical therapist in the state of Ohio. A physical therapist can treat without a prescription from a physician. This is called Direct Access. You can contact me directly to schedule an initial assessment. However, the name and contact information of your primary care physician, OBGYN, or midwife is required (on the Intake Form).

How do I schedule an appointment??

Please send me a message through the link provided on the Contact page or you can email me directly at restore.physicaltherapy614@gmail.com. Please provide a brief summary of what you wish to be seen for as well as a general day and time frame that would work for you. I do most of my scheduling via email. If you have additional questions or would like to speak over the phone prior to scheduling, please email me so we can set up a time for a phone call. 

What forms do I need to fill out prior to my appointment??

Once you have scheduled your appointment with me, you will receive an email from my online billing system with some forms to fill out prior to your session.

How do I pay for my appointment??

Restore Physical Therapy & Wellness is a cash-based practice, meaning your insurance is not billed directly. Forms of payment accepted include: cash, credit/debit card, and HSA. If you would like to submit to your insurance for potential reimbursement, an invoice can be provided upon request with all information necessary. You will then need to submit the invoice to your insurance company and they will reimburse you directly if applicable.
